I have been playing with the People categories and come across the following issue:
1-a)   I used Face Recognition to enter a new person (about 10 files of the same person).
1-b)   The first file entered became the default cover photo.
1-c)   I then selected Face Manager and selected another photo to become the cover photo.
1-d)   The cover photo in the people list remained the original default photo (i.e., the cover photo did not change).
1-e)   I then selected Edit Person and the new photo selected was the cover photo.
So basically, changing the cover photo changes in Edit but not in People.
I have also played about in Families and found the following:
2-1)   I created the same family name four times using the unique tab Family_1 Family_2 Family_3 and Family_4.
2-2)   I did not create them in a 1, 2, 3, 4 order but rather 2, 3, 1, 4.
2-3)   The sort order within the "Family" is the same as created i.e., 2, 3, 1, 4.
2-4)   The sort order of several families already created is alphabetical.
It would appear that sorting is alphabetical by family name but not within families.

Is this only when you use the FM to select a new cover photo or also when you select the cover photo in the more common way, in the Viewer? Does pressing <F5> after clicking on the person update the cover photo?

Groups and Families are sorted alphabetically, based on the full family name, including the prefix you add.
Try pressing <F5> after clicking into the Family View to force a reload of all families. Does this set the order straight?

This is when selecting a new cover photo in Face Manager. F5 had no effect. However after closing down and reopening the cover photo updated.

The family sort order does not change with F5 nor when iMach is closed down and reopened.

This issue with the cover file not updating has been resolved for IMatch 2028.1.10.
The browser was just to stubborn and preferred the cached cover image over the new one.

I cannot reproduce the problem with the family name.
Families are sorted by their name, not their tag.
Did you use Family_1, Family_2 etc. as the family name or tag?
If you always used "Family" as the name, IMatch will have assigned different tags, since tags must be unique.
I get this result (I've created the Miller family in the same order you used):



The People View currently sorts by the family name only. This makes the order in which families with the same name appear random (depending on how the sort algorithm is implemented, e.g. if it is stable).

I think it makes sense to also use the tag when the names are identical. This at least allows the user to configure the sort order via the tag, if desired.

I used the same family name for all four examples and in the tag repeated the family name with the _1 _2 etc appended to the name.

I will have another look today, it only takes a misplaced , or space to change the sort order.
