I also encounter a situation that I think is similar: the territorial organization in Spain would need four fields:

- Country (España)
--  Autonomous community (Andalucía)
--- Province (Cádiz)
----  Municipality (Jerez de la Frontera)

while the IPTC/XMP fields are three:

- Country (España)
-- State/Province (¿Andalucía? ¿Jerez de la Frontera? ¿Andalucía/Jerez de la Frontera?)
--- City.

So, some use the "State/Province" field for the Province, others for the Autonomous Community or they put both, which is a mess when a hierarchical organization is needed difficult to solve.

To make matters worse, those who use Lightroom find that the Country field appears as Country/Region, which sometimes results in "España/Andalucía".

I raise this question to the user community to know how they solve these problems in their respective countries.
